This award recognises innovative teams and individuals who have implemented strategies to promote wellbeing and reduce loneliness. Judges looked for clear evidence of how people receiving care are supported to maintain meaningful connections with their family, friends, and wider community.

A Story of Reconnection: A Client’s Journey Home
Our nomination centred on the story of our client, a gentleman with complex neurological needs whose life was transformed through thoughtful, collaborative care. After years in a specialist facility, they longed to return home. With the dedicated leadership of Care Manager Amy Watkins, and close coordination with his family and former care team, we made that transition possible.
The Client now lives with his daughter, her family, and his beloved dogs – surrounded by the people and comforts that matter most. His live-in carer provides not only practical support but also companionship, helping him manage daily routines, reduce anxiety, and stay socially engaged.
Rebuilding Independence Through Personalised Care
Like all Mumby’s care plans, it was designed around the client’s passions and preferences. This is someone who enjoys an active lifestyle. Daily walks, engaging with nature, and time in the garden – a space his late wife once tended, are now part of his everyday life. These moments aren’t just therapeutic; they’re deeply meaningful.
By tailoring support to their unique needs and values, we’ve helped them regain a sense of independence, dignity, and joy.
Why Reducing Social Isolation Matters
Social isolation is a growing concern across the UK. According to the Campaign to End Loneliness, around 3.83 million people (7.1% of the population) feel lonely “often or always”, and this figure has not returned to pre-pandemic levels.1 Among older adults, the trend is even more pressing: with the number of over-50s experiencing chronic loneliness being projected to rise to over 2 million this year.2
